How did Patient hear about The Enclave at Rye? Referred to by St. Luke’s Hospital

Details of Experience:
Umbert, or “Bert,” was welcomed to The Enclave at Rye Nursing and Rehabilitation on January 12th, 2019, from St. Luke’s Hospital. The reason for his hospitalization was due to an injury in a car accident, which resulted in a fracture of the lumbar vertebrae. Shortly after his arrival, he was warmly greeted by members of the interdisciplinary team including nurses, therapists, and doctors.

Rob, his occupational therapist, met Umbert and performed his initial evaluations as well as preliminary goals. He presented with muscle weakness, unable to move at all- which severely impacts his ability to independently perform basic activities of daily living (ADL’s). Short term goals were established to help him regain his strength, while our nursing staff carefully monitored the pain levels, to ensure that he can maximize his time during therapy.

As Umbert was improving in therapy, he realized that he wanted to engage in activities in between his therapy sessions. Umbert would soon begin to attend recreational activities in which he enjoyed. Aside from the fun that he had, he also met other residents with whom he had built a friendship. Even when he was discharged, he said that he would still be in touch with them and will come back to visit. These friendships had also majorly affected his growth in rehabilitation.

After two months, Umbert realized he was making a huge turn-around. Not only was he able to get out of bed on his own, but he was also walking by himself; even taking the stairs. He became independent in feeding and bathing himself. He was a new person!

On April 22nd, Umbert was discharged home. Before he had left, he wrote a beautiful letter thanking all the departments of The Enclave. To put it in a few short words, he writes “…this facility has gone above and beyond in making my stay and my experience extraordinary…”
