Details of Experience: 

Frank was admitted to The Enclave at Rye Rehabilitation and Nursing Center on October 6th, 2021, from New York-Presbyterian Hospital. The Enclave at Rye was recommended to Frank as the best place to recover and regain his independence.

Upon arrival, the concierge warmly welcomed Frank, and he was immediately evaluated by the interdisciplinary team, including nursing, rehabilitation, social services, concierge, and recreation departments. Frank and his family spoke with the concierge to find out his likes and dislikes and share any other information that would make his stay more pleasant.

Kristina, Occupational Therapist, and Lauren, Physical Therapist, met with Frank and performed their evaluation. Frank came from the hospital following two falls in which he fractured his sacrum. Due to the pain, Frank needed extensive assistance for all his activities of daily living (ADLs). In addition, Frank required assistance for bed mobility, transferring, and was not ambulatory. The plan was for Frank to return to being independent so that he could return home.

Dr. Abbey and the nursing team kept Frank’s pain under control, using pain medication, as well as ice packs and bio-freeze. The pain management allowed him to participate in all of his therapy sessions. Frank loved Italian food and was always overjoyed when his son brought him pasta, enjoying a nice dinner date together. Frank really enjoyed watching game shows with his new roommate/friend, and it was nice to see a new friendship develop so quickly.

In the gym, Frank took advantage of his time. He would frequent the new step machine, parallel bars, and the Omni- cycle. We loved watching him lifting weights and walking down the hall with his walker.

Six weeks later, with the help from the therapy team, Frank was totally independent with all his ADLs and bed mobility and transfers. He was walking over 175 feet and was even able to climb five stairs. Frank was pleased with his progress and was ready to return home. Before leaving, he exchanged phone numbers with his new friend and hoped for a dinner date together in the near future.

On November 23rd, Frank’s son came to pick him up, and they walked out together smiling! We wish Frank the best of luck back at home!

Details of Experience: 

Freddie was admitted to The Enclave at Rye Rehabilitation and Nursing Center on January 18th, 2021, from New York-Presbyterian Hospital. The Enclave at Rye was recommended to Freddie as the best place for him to heal and learn to walk with his prosthetics. 

Upon arrival, the concierge warmly welcomed Freddie, and he was immediately evaluated by the interdisciplinary team, including nursing, rehabilitation, social services, concierge, and recreation departments. Freddie and his family were put in touch with Todd Schaffhauser and Dennis Oehler from the CareRite Amputee Training and Rehabilitation Program, who educated them about the program and gave encouragement. 

Tim, Occupational Therapist, and Lauren, Physical Therapist, met with Freddie and performed their evaluation. Freddie presented as a bilateral amputee below the knee, following amputation due to gangrene. Freddie was totally dependent on activities of daily living (ADLs), transfers, and ambulation and would benefit tremendously from PT and OT. Dr. Abbey and The Enclave’s Wound Care Physician stayed on top of Freddie’s wound, helping the healing process along the way until Freddie would be able to get his prosthesis. 

Two weeks into Freddie’s stay, a care plan meeting was held discussing his progress and the plan ahead. 

Freddie enjoyed watching TV and spending time with his daughter when she visited. He participated in some recreational activities and stayed very positive throughout.  

After a few months, the wound was healed, and Freddie got measured and fitted for his prosthesis, and they were ordered. After the prosthesis arrived, Freddie restarted his PT for prosthetic training. He required increased assistance during ADLs using his new prosthetics and had increased muscle weakness and difficulty maintaining his balance. With the assistance of Todd and Dennis, Lauren and Tim retrained Freddie on how to ambulate with his prosthesis, and every small achievement was celebrated. What an accomplishment! Within a short time, Freddie was up and walking with a rolling walker.

After 9 months here, Freddie was ready to return home. We were all so happy about his progress, and we were all ecstatic about his discharge! On October 14th, we all gathered in the main lobby to escort our hero out to his car. With posters and music, we all wished Freddie the best of luck! It was a very emotional day for Freddie and the staff who loved him.

Jahsiel was admitted to The Enclave at Rye Rehabilitation and Nursing Center on July 13th, 2021, from White Plains Hospital. The Enclave at Rye was recommended to Daniel by his doctor as the best place for him to rehabilitate.

Upon arrival, Jahsiel was warmly welcomed by the concierge, and he was immediately evaluated by the interdisciplinary team, including nursing, rehabilitation, social services, concierge, and recreation departments.
Shortly after, a personalized treatment plan was created by the team.

Cynthia, his Physical Therapist, and Vanessa, Occupational Therapist, met with Jahsiel and performed their evaluation. Jahsiel had a stroke at home, which resulted in weakness in his left side, and made him wheelchair-bound. He had no mobility in his left hand, which made it very difficult for him to perform the regular daily activities, such as: transferring, dressing, and bathing. It was clear that with hard work in the gym, Jahsiel would be able to return home being able to continue leading his regular life.

Nursing and Dr. Abbey got Jahsiel’s pain under control, and with his family and friends visiting every day, Jahsiel had all the support he needed. Jahsiel settled in and was thriving in no time. His daily visits in the garden, along with relaxing with music outside, helped keep him uplifted.

Jahsiel was able to use our special Hydroworx machine. Seeing Jahsiel suited up in his swimsuit, ready to go on the underwater treadmill, made everybody smile. He started walking with a quad cane and used the Estim and mirror therapy to gain balance and endurance. After two months here, Daniel was completely independent and was able to walk up the 36 stairs to his house. He was able to open and close and use his left hand.

Upon discharge, Jahsiel was extremely happy with his recovery and with our staff that he left us a beautiful five-star review on google.

Details of Experience:
Daniel was admitted to The Enclave at Rye Rehabilitation and Nursing Center on July 5th, 2021, from Greenwich Hospital. The Enclave at Rye was recommended to Daniel by the hospital, and after his daughter heard amazing things, the Enclave was chosen as the best place for Daniel to rehabilitate. Upon arrival, Daniel and his daughters were welcomed by the Concierge, and he was immediately evaluated by the interdisciplinary team, including nursing, rehabilitation, social services, Concierge, and recreation departments. Shortly after, a personalized treatment plan was created by the team.

Meaghan, his physical therapist, and Raquel, his occupational therapist, met with Daniel and performed their comprehensive evaluation. Daniel had a fall at home, which caused weakness in both legs, especially on his entire left side. Daniel required maximum assistance with transferring and was non-ambulatory. With regard to his ADLs, Daniel needed help with dressing, bathing, and toileting. It was obvious that Daniel would benefit from physical and occupational therapy.

Nursing and Dr. Abbey were able to get Daniel’s pain under control, and with good communication with his family, Daniel settled in very nicely. It was suggested that his Daniels daughters be present during therapy to help encourage a strong performance. Daniel loved to sit out in the garden with his newly found friends and chat and relax under the sun. One day, Daniel’s family even came with some gifts for some of Daniel’s friends.

Daniel would enjoy eating snacks from the kitchen, and he always enjoyed it when his daughter would bring him some Burger King. Daniel worked himself hard in the gym. He trained with the new-step machine, which helped him with his endurance, and strengthened his arms and legs. The parallel bar was used for standing, and dumbbells were used for strengthening.

After just a few weeks here, Daniel was able to transfer out of bed by himself, toilet on his own, as well as get dressed without assistance. When he arrived, he was unable to walk more than five feet, and at discharge, he was able to walk over 150 feet with a walker. A few weeks and a total transformation!

Daniel was picked up by his daughter on August 12th and was brought home, where his family surprised him with a bedroom makeover. Following the discharge, our Concierge reached out to Daniel’s family to see how he was doing. They responded that he is doing well, but he misses being social with the friends that he made over at the Enclave!

Details of Experience:
Gerald was admitted to The Enclave at Rye Rehabilitation and Nursing Center on June 22, 2021, from Greenwich Hospital. The Enclave at Rye was recommended to Gerald by the hospital as the best place for Gerald to get back on his feet. Upon arrival, Gerald was welcomed by the concierge and was immediately evaluated by the interdisciplinary team, including nursing, rehabilitation, social services, concierge, and recreation departments. 

Shortly after, the team created a personalized treatment plan, and Gerald would greatly benefit from physical and occupational therapy, as well as getting on a balanced diet. Robert, his physical therapist, and Andrea, occupational therapist, met with Gerald and performed their comprehensive evaluation. Gerald suffers from Parkinson’s disease, and one day at home, he began to feel weak, which ended him up in the hospital with bilateral leg swelling and general weakness. Upon evaluation, Gerald was no longer able to transfer out of bed alone, was unable to do his activities of daily living (ADL’s), including bathing, dressing, and toileting. He also presented with malnutrition and was only able to ambulate 20 feet.

Gioia, our dietician, worked diligently with Gerald ensuring that the proper diet for him was maintained, as nursing and Dr. Abbey made sure his medications were administered on time. As Gerald started regaining his strength, he participated in recreation and enjoyed watching some of his favorite game shows. Gerald would talk and joke around with the concierge, and he especially enjoyed it when his daughter came to visit.

Gerald worked himself hard in the gym. He would train with the omni-cycle, and then he built up to the new-step machine, which helped him with his endurance. The parallel bar was used for standing, and dumbbells were used for strengthening.

After just 3 weeks here, Gerald was able to transfer out of bed by himself, and he was able to toilet on his own and get dressed without assistance. When he came in, he was able to ambulate just 20 feet, and at discharge, he could ambulate over 350 feet!  Gerald worked hard, and his work paid off! On July 18, Gerald was discharged back home feeling and looking great!

Dorothy was admitted to The Enclave at Rye Rehabilitation and Nursing Center on April 23, 2021, from Greenwich Hospital. The Enclave at Rye was recommended to Dorothy by her doctor after an exacerbation of COPD.

Upon arrival, Dorothy was welcomed by the concierge and was immediately evaluated by the interdisciplinary team, including nursing, rehabilitation, social services, concierge, and recreation departments. Shortly after, a personalized treatment plan was created by the team. Dorothy would benefit from physical and occupational therapy. 

Robert, her physical therapist, and Vanessa, her occupational therapist, met with Dorothy, and they got to work. Because of the COPD, Dorothy was having a hard time breathing, and she was on oxygen all day. She required maximum assistance with all of her ADL’s, as well as, she couldn’t walk or transfer. Her therapists were going to work extra hard so that Dorothy could return home fully functional.   

Dr. Abbey and the nursing team worked with Dorothy monitoring her oxygen and trying to stabilize her breathing. Dorothy was a regular in the day room, where she enjoyed her morning coffee with a pastry. She participated in all the recreational activities, and she enjoyed a special bond with our Concierge, Moshe. Dorothy’s daughter would come and visit regularly and sip some Dunkin Donuts coffee with mom. We loved watching the bonding at these visits.

Dorothy’s therapists and the nursing team were able to work their magic on Dorothy. They completely got Dorothy off of oxygen, and Dorothy could walk with a rollator walker over 350 feet! She became completely independent with all her ADL’S including bathing and using the bathroom.

On June 19th, Dorothy was discharged back home feeling and looking great. Dorothy loved all the staff and commented how much she would miss all of us!

Michael was admitted to The Enclave at Rye Rehabilitation and Nursing Center on May 8, 2021, from Greenwich Hospital. The Enclave at Rye was recommended to Michael by his doctor, following his hip replacement surgery.

Upon arrival, Michael was welcomed and evaluated by the interdisciplinary team, including nursing, rehabilitation, social services, Concierge, and recreation departments. Shortly after arrival, a personalized treatment plan was developed by the team. Michael would benefit from physical and occupational therapy. 

Robert, his physical therapist, and Frishta, his occupational therapist, met with Michael, and they got to work. Following Michael’s hip replacement surgery, Michael was experiencing muscle weakness and pain in his hip. He required physical therapy to regain strength, improve bed mobility, and help him walk and ambulate stairs again. Occupational therapy was to help him with his ADLs, including bathing, toileting, and dressing.   Dr. Abbey and the nursing team worked closely with Michael monitoring his pain while making sure his medications were dispensed on time.  

Michael enjoyed hanging out in the day-room drinking coffee while socializing with the other residents, all the while listening to some hits from the ’60s. We also had the pleasure of hosting Michael’s new baby granddaughter, as well as his cute puppy.  After just two weeks here, Michael was walking and going upstairs again. His mobility and bed transfer abilities had improved beyond expectation. As his pain subsided, Michael was ready and excited to go back home. Janel, his social worker, arranged for all of the equipment that Michael would need to be delivered to his home.

On May 24, Michael shared that he really appreciated the staff, and he was extremely happy that he chose the Enclave.  Michaels, son came to pick him up, and he walked out of the Enclave as a friend!

Details of Experience:
Constance was admitted to The Enclave at Rye Rehabilitation and Nursing Center on March 2, 2021, from the Hospital.  The Enclave at Rye was recommended to Constance by her doctor at the hospital for rehabilitation.

Upon arrival, the interdisciplinary team greeted and evaluated Constance, including nursing, rehabilitation, dietary, social services, concierge, and recreation departments. Within 24 hours of Constance’s arrival, a personalized treatment plan was created by the team. Constance would benefit from physical and occupational therapy. 

Lauren, her physical therapist, and Tim, her occupational therapist, met Constance and performed their initial evaluations and preliminary goals. She presented with balance deficits, gross motor coordination, and muscle weakness, severely impacting her ability to independently perform basic activities of daily living (ADL’s). She required physical assistance to ambulate 20 feet and needed extensive assistance with dressing, bathing and toileting. Goals were established to help her regain strength, while nursing carefully monitored the pain levels so that she could maximize her time in therapy.

Some of the goals included performing bed mobility tasks with Independence, ambulating 300 feet with a walker, and ascend/descend 15 stairs with modified independence.  The nursing department kept a close eye on Constance’s pain management, and managed her high blood pressure, and provided medical management for DVT, enabling her to maximize her time in therapy. Constance enjoyed the recreational activities such as music, puzzles, games, and socializing with the other residents in the day room. 

After six weeks here, Constance’s progress was remarkable. She met and surpassed all of her goals, took care of all ADLs, and was ready to adjust back to life at home. On April 14, Constance walked out of the Enclave with renewed strength and independence.

Her confidence in her health and independence was at a new high, and she was super excited to go back to her community. All the residents now miss her smile and upbeat presence!

Short Term Rehabilitation Following Fall, Hemorrhage, and Altered Mental Status

Details of Experience:
Willie was admitted to The Enclave at Rye Rehabilitation and Nursing Center on December 2nd, 2020, from Burke Rehabilitation Hospital. Willie was at home and started to feel light-headed, resulting in him falling and hitting his head against the wall. Willie was found to have a right thalamic hemorrhage at the hospital, with lateral and third ventricular extension. His hospital course was complicated by altered mental status (AMS) and chest pain. The Enclave at Rye was recommended to Willie by his doctor at the hospital for rehabilitation.

Upon arrival, Willie was greeted and evaluated by the interdisciplinary team, including nursing, rehabilitation, dietary, social services, concierge, and recreation departments. Within 24 hours of Willie’s arrival, the team created a personalized treatment plan. Willie would benefit from physical, occupational, and speech therapy. Upon evaluation from the Occupational Therapists, Willie was found to require maximum assistance for most activities of daily living (ADL’s) and functional mobility, and he needed maximum assistance to maintain standing balance. A personalized treatment was created to address all ADL’s.

Goals for his physical therapy were created to address his standing, transferring, walking, and ascending/descending steps. His short-term goals were to improve his standing balance and to walk 50 feet with maximum assistance. His long-term goals were to ascend and descend 15 steps and ambulate 350 feet.

The nursing department kept a close eye on Willies pain management, enabling him to maximize his time in therapy. Willie loved to order from the always available menu, and the chef loved going the extra mile to accommodate him. Willie loved to strike up a conversation with the nursing staff and the concierge, and he enjoyed the movies and the social hour provided by the recreation department. After one month here, Willie was progressing rapidly. He was able to take care of all his ADLs with minimum assistance, standing balance without support, and ambulate 100 feet with a rolling walker.

When Willie was ready for discharge, he was transferring independently and able to ascend/descend more than 15 steps with no physical assistance and walking with a walker over 250 feet. He was able to complete all ADL’s independently.

On March 12th, Willie walked out of The Enclave with renewed strength and independence. His confidence in his physical independence was at a new high!
