Details of Experience:
Sarah was admitted to The Enclave at Rye Rehabilitation and Nursing Center on December 9, 2020, from White Plains Hospital. The Enclave at Rye was recommended to Sarah by her doctor. Sarah has stage 4 bladder cancer, which spread to the pelvic floor muscles causing her pain in walking and standing. Upon arrival, Sarah was greeted and evaluated by the interdisciplinary team, including nursing, rehabilitation, dietary, social services, concierge, and recreation departments.

The Rehabilitation team developed a personalized treatment plan for Sarah within 24 hours of her arrival. Sarah required maximum assistance while being transferred, was not ambulatory, and could not perform Activities of Daily Living (ADLs). Starlyn Adames, the Occupational Therapist, met with Sarah. Based on his assessment, it was clear that Sarah would benefit from restorative Occupational Therapy to minimize impairments, increase activity tolerance and strength, and increase independence with ADLs to return to her PLOF(prior level of function). After four weeks of intensive balance and tolerance activity training, Sarah progressed at a rapid pace. She saw improvements in her mobility and her pain was decreasing. Cynthia James, Physical Therapist, met with Sarah, developing short-term and long-term goals for Sarah, including performing bed mobility tasks with minimal assistance and safely walk 10 feet using a walker. Her long-term goals included performing bed transfers independently, and to be able to walk 50 feet with a walker. After just three weeks, Sarah had reached 30 feet with a walker. This progress propelled Sarah to continue working hard.

Sarah received close psycho-social support from the Concierge and Recreation departments. Moshe, the Concierge shared one-on-one time with Sarah, which included Sarah beating him at Tic Tac Toe. Recreation presented independent activities to Sarah daily, and she especially liked playing bingo with Amberly.

After spending less than two months here, Sarah was discharged being able to walk 75 feet with a walker, as well as perform all ADL’s independently. Social Work partnered with Sarah’s daughter to ensure Sarah would have continued support at home. Social Work reached out to Visiting Nurse Service of NY to coordinate home care, as well as order adaptive equipment Sarah needed to be safe at home. On January 20th, Sarah’s daughter greeted her at home, ecstatic to be able to have her mother home safely.

The team at The Enclave was honored to play a part in Sarah’s journey to recovery.

Ernest was admitted to The Enclave at Rye Rehabilitation and Nursing Center on October 5, 2020, from Yale New Haven Hospital, following a surgical procedure. Shortly after his arrival, he was warmly greeted by members of the interdisciplinary team, including the nurses, therapists, and doctor.

Mylene, the Director of Rehabilitation, and Tim, the Assistant Director of Rehabilitation met Ernest and performed their initial evaluations as well as preliminary goals. He presented with muscle weakness, needing assistance to rise from his chair; due to the fact that it took a couple of times for him to rise himself. He also needed help to balance his standing. All of this severely impacted his ability to independently perform basic activities of daily living (ADL’s). Short term goals were established to help him regain strength while nursing carefully monitored the pain levels so that he could maximize his time in therapy.

Only two weeks later, Ernest was realizing that he was progressing at a quick pace. He was so thrilled that his discharge date was coming sooner than later. As he was talking to the Director of Concierge Services, he acknowledged the great work that everyone has done for him, stating that everyone was “absolutely the best.” He specifically was thankful for the excellent rehabilitation team; as well as the nurses who “made him better.”

On October 22, only 17 days after he was admitted, Ernest was discharged home with renewed strength and independence. He walked to the care that picked up without ANY assistance. His confidence in his physical independence was at a new high!

Margarita Alvarez is someone with a remarkable story. She was hospitalized at Greenwich Hospital from an Acute Kidney Injury. She was told at the hospital, because of her injury, her life has been greatly affected and she only had a few weeks to live. But Margarita, although she was scared and depressed, did not give up hope. Margarita was admitted to The Enclave at Rye Rehabilitation and Nursing Center on July 13, 2020 from Greenwich Hospital. Shortly after her arrival, she was warmly greeted by members of the interdisciplinary team, including the nurses, therapists, and doctor.

Sheldon, her physical therapist, and Tim, her occupational therapist, met Margarita and performed initial evaluations as well as set preliminary goals. She presented with muscle weakness, unable to turn 360 degrees, or even balance her standing. She needed maximum assistance to stand and sit. All of this has severely impacted her ability to independently perform basic activities of daily living (ADL’s). Short term goals were established to help her regain strength while nursing carefully monitored any signs of symptoms, so that she could stay healthy and maximize her time in therapy.

After two weeks, Margarita was feeling completely different. She was improving with physical and occupational therapy at a rapid pace. Before she knew it, she was walking without any assistance. She was able to safely sit and stand independently. Her standing balance was steady, and she was able to turn 360 degrees with continuous steps. Everywhere she went, the smile on her face never faded. She was not only happy herself, but she made those that surrounded her cheerful as well.

On August 6, Margarita was discharged home. When her daughter came to pick her up, it was the first time she had seen her in many weeks due to the Coronavirus. When she finally saw her mom, she was crying out of pure joy. She was ecstatic on the appearance of her mother who was told that her end was near. Margarita was very happy with her experience at The Enclave. A few days following discharge, she asked if she could come back and visit.

Albert was admitted to The Enclave at Rye Rehabilitation and Nursing Center on April 29, 2020, from Yale New Haven Hospital, following an inpatient stay due to contraction of COVID-19. Shortly after his arrival, he was warmly and carefully greeted by specific members of the interdisciplinary team, including the nurses, therapists, and doctors.

Salazar, his physical therapist, and Mylene, his occupational therapist, met Albert and performed their initial evaluations as well as preliminary goals. He presented with muscle weakness, unable to sit straight, stand up, or stand straight. He was completely immobile. Due to the infliction of the virus, he was completely dependent when using the bathroom, getting dressed, using the stairs, and bathing. All of this has severely impacted his ability to independently perform basic activities of daily living (ADL’s). Short term goals were established to help him regain strength while nursing carefully monitored any signs or symptoms so that he could stay healthy and maximize his time in therapy.

When Albert first arrived at The Enclave, he shared he was distraught. The virus had been a tremendous setback, and it seemed like it was going to take a long time for him to get back to his normal life. But after two weeks, Albert showed great improvement. He was satisfied with the care he received and he realized that he was improving in therapy much faster than he expected.

After three weeks, Albert shared he felt like a new man; physically and emotionally. He became independent for all of his ADLs, and his smile came back again. He said that everyone took such good care of him, and the therapy has been “so much fun and enthusiastic”. When asked if he can rate his overall stay from 1-10, he said that it should be a “14”.

On the day that Albert was discharged, the staff created a beautiful sendoff for Albert leaving the building. The team experienced joy, as Albert and his wife waltzed their way into their car, returning home with renewed hope and strength. The team at the Enclave was honored to be a part of Albert’s recovery journey and wishes him the best.

Alicia was admitted to The Enclave at Rye Rehabilitation and Nursing Center on April 21, 2020, from Greenwich Hospital, where she had been admitted due to a Coronavirus diagnosis. Alicia was greeted by The Enclave’s team, led by her attending team of nurses, therapists, and doctors. Upon admission, Mylene, her physical therapist, and Dave, her occupational therapist, had the opportunity to meet with Alicia and perform their initial evaluations. In this time, they created her preliminary goals and personal treatment plan.

At admission, Alicia was presenting with muscle weakness and needed support while standing, along with an inability to turn around fully with continuous steps. During the duration of her Coronavirus treatment, she needed assistance walking, using stairs, and getting dressed. Her muscle weakness and diagnosis impacted her ability to complete basic daily living activities, so short term goals were established to help her regain strength while monitoring her symptoms and health.

Staff had been informed prior to Alicia’s admission that she was primarily Spanish speaking. As such, the staff took extra steps to ensure Alicia would still receive the highest quality of care and understand her treatment plan. Only a week into her stay, Alicia stated, “They are taking good care of me”. Along with Alicia’s satisfaction in nursing care, she was thrilled with how therapy was progressing. In her own words, “Everybody was so nice and patient with me!” With all the wonderful care she was receiving, Alicia was progressing towards her short term and long term goals.

Only two weeks after joining The Enclave, Alicia was discharged back home with renewed strength and independence after crushing COVID-19! She needs no support when standing, and can turn 360 degrees with continuous steps. She was able to walk and use the stairs by herself, as well as dressing herself. She was independent in all of her daily living activities, including her bathing and toileting.

The Enclave’s team is so thrilled that Alicia has returned home. When she was asked about her experience at The Enclave, she gave the community a rating of 10 out of 10, stating that “I was very impressed”. The team wishes Alicia the best of luck, and continued health!

What seemed like an enjoyable Spring and Summer ahead for the Burchman family quickly took a turn for the worse when 63-year-old Dr. Albert Burchman fell ill and was emergently admitted to Greenwich Hospital in Greenwich, Connecticut for fever and increased weakness.

Shortly after her arrival, she was warmly greeted by members of the interdisciplinary team, including the nurses, therapists, and physicians.

Cynthia, her physical therapist, and Tim, her occupational therapist, met Dorothy and performed their initial evaluations as well as preliminary goals. She presented with muscle weakness, decreased sitting balance and immediate standing balance, and limited turning of 360 degrees, severely impacting her ability to independently perform basic activities of daily living (ADL’s). She was wheelchair independent and needed help with dressing and bathing. Short term goals were established to help her regain strength while nursing carefully monitored the pain levels so that she could maximize her time in therapy.

The day after Dorothy was admitted into The Enclave, she realized that her good friend was coming to The Enclave. She had notified the staff about this. Without a moment’s delay, the Admissions team along with Nursing and Maintenance arranged a room change so that Dorothy can reside with her good friend. Needless to say, they were both thrilled to hear this. This also got Dorothy to a good motivational start in Therapy.

Only one month after entering The Enclave, Dorothy was discharged, together with her friend, back home with renewed strength and independence. Her sitting balance and immediate standing balance was steady. She was able to walk continuous steps to turn 360 degrees. She was independent in her bathing and dressing, and most important of all, she was not wheelchair-bound. As a matter of fact, she was able to walk on her own two feet. Amazing!!

We are so thrilled that Dorothy is back home. When she was asked about her experience here, she gave a rating of 10/10, saying how the staff was “exceptional, and very nice”! We wish Dorothy the best of luck!

Donna was admitted to The Enclave at Rye Rehabilitation and Nursing Center on June 27, 2019, from Greenwich Hospital, following an inpatient hospitalization due to an acute gastric ulcer. Shortly after her arrival, she was warmly greeted by members of the interdisciplinary team, including the nurses, therapists, and on-staff physician.

Sheldon, her physical therapist, and Tim, her occupational therapist met Donna and performed their initial evaluations as well as preliminary goals. She presented with muscle weakness, totally dependent on a wheelchair for mobility, and completely required assistance for her daily needs, such as dressing and using the bathroom. All of this severely impacted her ability to independently perform basic activities of daily living (ADL’s). Short term goals were established to help her regain strength while nursing carefully monitored the pain levels so that she could maximize her time in therapy.

As Donna was improving in therapy, she was also being quite supported by the clinical team here at The Enclave. Often, Donna’s head would slide down from the pillow, and because of her weakness, she wasn’t able to slide back up. This was extremely frustrating for her, as she had to ask for assistance multiple times of the day. However, with the support of the C.N.A.’s along with Concierge Services, she didn’t have to worry about a thing. She was always being helped with a smile, and that helped her continue to excel throughout her stay.

Less than a month after arriving at The Enclave, Donna was discharged back home with renewed strength and independence. She became independent in everything that she worked on. She was able to use the bathroom by herself. She was able to dress. And most of all, she was able to walk around without having to use a wheelchair. Because she was already a patient once at The Enclave, Donna says “There is a reason why I came back!”
