Mr. G. was admitted to The Enclave at Rye Rehabilitation and Nursing Center on March 8th, 2023, from White Plains Hospital. The Enclave at Rye was recommended to Mr. G. by his doctor at the hospital and was selected after reading the online reviews.

Upon arrival, Mr. G. was welcomed in by the concierge and evaluated by the interdisciplinary team, including nursing, rehabilitation, social services, and recreation departments. Mr. G. presented with a diverse medical history, including a history of hypertension, a prior stroke resulting in residual weakness, vascular dementia, throat cancer, subarachnoid hemorrhage, and a seizure disorder. Prior to admission, Mr. G. had been enjoying life living at home with his wife, but after multiple falls and declining physical function, he required the assistance of two to help with transferring and mobility. Our Physical therapists developed an individualized exercise program to improve lower extremity strength, balance, and mobility. Progressive strength training exercises, gait training, and balance activities were implemented. Through these interventions, Mr. G. made steady progress.

In addition, Julie, our Speech-Language Pathologist played a pivotal role in enhancing the cognitive skills of Mr. G. She employed speech therapy techniques to promote communication and improve cognitive function. The therapy focused on speech for cognition, utilizing activities to enhance verbal problem-solving abilities and promote safety awareness. Additionally, compensatory strategies were introduced to aid in day-to-day functioning.

The Enclave features a nurturing environment where Mr. G. actively participated in recreational activities and engaged in meaningful interactions with staff and fellow patients. The regular involvement of his family in his care greatly contributed to his emotional well-being and recovery. The support and companionship he received from both staff and family fostered a positive healing atmosphere. Throughout his rehabilitation journey, Mr. G. demonstrated remarkable progress. His lower extremity strength and standing balance significantly improved, allowing him to regain independence in activities of daily living (ADLs), in addition to being able to ambulate safely with the assistance of just a cane. Furthermore, his attention and cognitive function showed notable enhancements, resulting in increased accuracy during tasks. These advancements not only boosted his confidence but also positively impacted his overall quality of life. Mr. G. and his family were overwhelmed with joy and gratitude for the exceptional care and support provided by the Enclave staff. To express their appreciation, they organized a catered lunch for all the staff, a testament to the positive impact the team had on their lives.

This case study exemplifies the successful rehabilitation journey of a complex patient with multiple comorbidities. The multidisciplinary approach, combining physical and cognitive rehabilitation interventions, along with a supportive and engaging environment, played a significant role in Mr. G.’s exceptional recovery.
