Details of Experience:
Dr. M was admitted to The Enclave at Rye Rehabilitation and Nursing Center on January 30th, 2025. Upon arrival, Dr. M was welcomed by the concierge and introduced to the interdisciplinary care team.
The Interdisciplinary team, including Nursing, Rehabilitation, Social Services, and the Recreation department, immediately completed their evaluation. Once all evaluations were completed, he was ready to start his therapy journey and resume his life with better health and complete independence.
Dr. M is a 74-year-old male with a history of coronary artery disease. He was admitted after a long hospital stay due to a complicated operation, which required multiple emergent surgeries for hemorrhagic shock and ventricular fibrillation arrest, including reopening of the sternotomy, graft repair, and open cardiac massage. In our community, he was a candidate for rehab for functional mobility, progressive ambulation, balance, motor control, strengthening, and endurance.
During the first weeks of his admission, Dr. M presented with a deficit in muscle strength, balance requiring some assistance with all his activities of daily living (ADL’S) and all transfers- such as wheelchair to bed, etc. Dr. M was very motivated to start therapy. He quickly developed upper- and lower-body strength and improved his transfers and mobility. He made good progress with ADLs, driven by gains in muscle strength, endurance, activity tolerance, and balance. He achieved his highest practical level in therapy and was ready for discharge.
After just a month of admission, he was ready for discharge; he was fully independent. The team was so proud of his progress and his success. We all gathered in our lobby, wishing him all the best in his new journey. Dr. M returned home and resumed work after his discharge.
